The layout lines up with the parameters set by UNIVERSAL / ATTACHE / ARROW / CBA for their invoice set up. 

This form is a 3 ply product, so allows for a copy to be sent to the customer and 2 further copies to either be kept internally or to be given to another department/party for record keeping.
