For use in dot matrix (tractor feed) printers, this pre-printed form will enable the user to print professional looking statements with their IAL/CBA/ARROW Software package.

The layout lines up with the parameters set by the stated software packages for their statement set up. 

This form is 1 ply, but has a vertical perforation on the left hand side of the form, so that a copy can be kept for office records and the main bulk of the form can be sent to the customer.
